About the role

To empower patients on their wellness journey, the full-time Board Certified Health Coach will provide remote consultations, implement behavior change strategies, and support personalized care plans while collaborating with interdisciplinary teams.

Key responsibilities: Implement coaching techniques and behavior change solutions to improve patient outcomes Conduct video consultations and messaging with patients to support their wellness goals Collaborate effectively with cross-functional teams to ensure optimal patient-centered care

Required qualifications: Board Certified Health and Wellness Coach Proficiency in motivational interviewing and establishing SMART goals with patients Minimum of 2 years of experience as a health coach or wellness educator Demonstrated competence in digital care delivery or telehealth Strong communication skills and ability to manage a patient caseload effectively
