About the Role:
A well-established, physician-led outpatient group on Long Island is seeking a skilled Internal Medicine Nurse Practitioner to join their team. The practice is known for clinical excellence, patient satisfaction, and strong collaboration across disciplines. This is a fully outpatient role with no on-call responsibilities and a flexible schedule.
Key Responsibilities:
- Deliver high-quality patient care in an outpatient primary care setting, managing preventive, acute, and chronic conditions
- Refer patients to appropriate specialty services as needed
- Educate patients about diagnoses, treatment plans, and preventive health strategies
- Maintain accurate and thorough documentation in the EMRCollaborate with physicians, specialists, and care teams to ensure seamless patient care
- Provide clinical oversight and mentorship to nursing and support staff
- Ensure compliance with HIPAA, NYS DOH, CMS, and payer guidelines
- Participate in population health initiatives and quality improvement projects
- Support the growth and management of a sustainable patient panel
Qualifications:
- BSN and MSN from an accredited nursing programNYS RN & NP license, BLS, and Board Certification in Internal Medicine
- Strong clinical judgment, communication, and interpersonal skills
- Team-oriented, patient-centered approach to care
- Familiarity with EMR systems and value-based care metrics a plus
- Entrepreneurial mindset and interest in growing a thriving patient panel
