Home Health and Hospice Clinical Liaison RN

Gentiva is seeking an RN Clinical Liaison to support patients transitioning to home-based care. In this role, you will educate patients and families on Gentiva’s home health and hospice services, assess clinical needs, coordinate referrals, and collaborate with hospitals, physicians, and care teams. You will complete timely assessments, develop care transition plans, and ensure smooth admissions to home-based services. The RN Clinical Liaison serves as a clinical resource and advocate, promoting compassionate, dignified care while building strong community and provider relationships in a mission-driven environment. Assess patients for eligibility and appropriateness for home health and hospice services. Educate patients, families, and referral sources on Gentiva’s services and care options. Coordinate referrals and admissions from hospitals, facilities, and physician offices. Collaborate with physicians and interdisciplinary teams to develop transition plans. Complete timely and accurate clinical documentation and assessments. Serve as a clinical resource and advocate for patients and families. Build and maintain relationships with referral sources and community partners. Support seamless care transitions to promote safety, comfort, and dignity at home. Participate in outreach and education to grow awareness of Gentiva’s services. Ensure compliance with regulatory and organizational clinical standards.
