We are seeking an experienced and compassionate full-time RN to join us in this mission. Job DescriptionRN Hospice Case Manager Performs comprehensive nursing evaluations. Assesses for signs and symptoms of discomfort including presence of pain and takes prompt and appropriate action. Initiates and manages plan of care, provides appropriate nursing follow –up to all patients, and instructs caregivers in managing patient needs. Coordinates all services provided to patients and families, as well as appropriateness of care, to ensure quality of care and services.
