Alzheimer's Care Nurse Practitioner or Physician Assistant

Join the South Carolina Alzheimer's Network to provide specialized care for patients with Alzheimer's disease and other cognitive disorders. Evaluate and manage patients, develop individualized treatment plans, provide longitudinal follow-up, and collaborate with interdisciplinary teams. Educate patients and caregivers and participate in clinical research and quality improvement. Evaluate, diagnose, and manage Alzheimer's disease and other cognitive and behavioral neurological conditions. Conduct comprehensive patient histories, physical examinations, and diagnostic evaluations. Develop individualized treatment plans and provide longitudinal follow-up care. Coordinate care with neurologists, advanced practice providers, primary care clinicians, and interdisciplinary teams. Educate patients and caregivers about disease management, treatment options, and resources. Participate in clinical research, quality improvement, and educational activities.
