Palliative Care RN: Telehealth Care Coordinator

Big Bend Hospice in Tallahassee, FL is seeking a Registered Nurse Case Manager - Palliative Care to support outpatient palliative care through telephonic nursing, care coordination, patient triage, and ongoing case management. The RN will work with palliative care providers, patients, caregivers, primary care providers, specialists and community partners to address clinical needs, close gaps in care, ensure timely access to services, and document PCM activities for compliance and billing.
