Registered Nurse - Home Health Schedule8hrs Assignment Details Guaranteed Hours: 40 Contract Weeks: 14
Requirements:
- Current state license as a Registered Nurse - Required
- Homecare Homebase Software Experience - RequiredOASIS Experience - Required 2+ years Home health experience - Required 1-2 years experience as a Registered Nurse in a clinical care setting - RequiredCHPN certification - Preferred
- Proof of current CPR certification - Required
- Current driver's license - Required
- Must have reliable transportation and insurance - Required
- Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program - Required
- Bachelors of Science in Nursing - Preferred
Responsibilities:
Assess home health and/or hospice patients to identify their needs.
Implement and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
Educate patients and families regarding care, medications, and safety.
Provide clinical direction to support staff to ensure quality service.
Monitor cases for compliance with third-party payer requirements.
Facilitate communication among patients, families, and healthcare professionals.
Perform on-call responsibilities as assigned.
