Job DescriptionNOW HIRING: Full-Time Caregiver – Holland, MIContinuum Home Health Care is growing, and we’re looking for a dependable, compassionate, and professional caregiver to join our Holland-area team. Position Details
Location: Holland, Michigan
Hours: Approximately 35 hours per week
Schedule: Full-time
Weekend Requirement: Every other weekend availability is required Day, evening, and weekend availability may be needed based on client schedules Consistent attendance and reliability are extremely important
What You’ll Be Doing: As a caregiver with Continuum Home Health Care, you’ll provide one-on-one support to clients in their homes. Your responsibilities may include:
  • Assistance with bathing, dressing, grooming, and personal hygiene
  • Toileting and incontinence care
  • Mobility assistance and safe transfers
  • Meal preparation and feeding assistance
  • Medication reminders
  • Light housekeeping and laundry
  • Companionship and social interaction
  • Grocery shopping and errands
  • Transportation to appointments when needed
  • Providing supervision and safety support
  • Documenting care provided during each visit
  • Communicating changes in a client’s condition to the Continuum team
  • Supporting clients living with dementia, Alzheimer’s disease, Parkinson’s disease, mobility limitations, or other age-related needs

Requirements:
  • Valid driver’s license
  • Reliable transportation
  • Current auto insurance
  • Ability to pass required background checks
  • Dependable phone and communication
  • Ability to safely assist clients with mobility and activities of daily living
  • Previous caregiving, home care, CNA, resident aide, or direct care experience is preferred
  • Dementia care experience is a plus
