Hospice Registered Nurse Care Manager

Heartland Hospice, part of ProMedica, seeks an Afterhours RN Care Manager to provide triage, crisis intervention, and skilled nursing care to hospice patients during evenings, nights, weekends, and holidays. The RN coordinates symptom management, pain control, and emergency visits in home and facility settings, ensuring comfort, dignity, and support for patients and families. Responsibilities include phone triage, assessment, implementing care plans, collaboration with the interdisciplinary team, accurate documentation, and providing emotional education and support aligned with hospice philosophy and regulatory standards. Provide afterhours hospice triage and crisis intervention via phone and in-person visits Perform comprehensive nursing assessments and manage pain and symptoms Implement and adjust care plans in collaboration with the interdisciplinary team Coordinate care with physicians, social workers, chaplains, and aides Educate and support patients and families on disease process and end-of-life care Document assessments, interventions, and communications accurately and timely Ensure compliance with hospice policies, procedures, and regulatory standards Participate in on-call rotation for evenings, nights, weekends, and holidays Communicate changes in patient condition to case managers and leadership Promote a patient-centered, compassionate, and dignified care experience Hospice nursing Telephone triage Pain and symptom management Comprehensive patient assessment Electronic medical record (EMR) documentation Care planning and coordination Crisis intervention End-of-life patient and family education Interdisciplinary team collaboration Knowledge of hospice regulations and standards Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active RN license in state of practice Current BLS certification Hospice and Palliative Care Nursing Certification (CHPN) preferred Experience in hospice, palliative care, oncology, or home health preferred Valid driver’s license and reliable transportation
