Overview:
Phoenix Mountain Therapy is seeking a passionate Physical Therapist to join our growing outpatient therapy team serving Assisted Living and Independent Living communities throughout the Chandler area. This is an excellent opportunity to provide one-on-one outpatient therapy services while making a meaningful difference in the lives of older adults. Whether you're an experienced clinician or a new graduate looking to expand your skills, you'll receive mentorship, clinical support, and opportunities for professional growth within a collaborative, therapist-led organization. What You'll Do:
- Evaluate and treat patients with orthopedic, neurological, balance, gait, mobility, and functional impairments
- Develop individualized treatment plans using evidence-based practice
- Help patients improve strength, mobility, balance, endurance, safety, and overall functional independence
- Collaborate with physicians, community staff, families, and interdisciplinary team members
- Educate patients and caregivers on treatment plans and home exercise programs
- Complete timely and accurate documentation
Qualifications:
- Current Arizona Physical Therapist license (or eligibility for licensure)
- Strong communication and interpersonal skills
- Passion for serving the geriatric population
- Interest in professional growth and providing exceptional patient care
