Licensed Professional Counselor - Hybrid

What You'll Do: Therapeutic Interventions: Provide evidence-based therapy focusing on behavior modification, coping strategies, and emotional health. Assist patients in processing trauma and building resilience. Integrated Care Collaboration: Collaborate with physicians, care managers, and primary care staff to create and maintain comprehensive care plans. Offer insights into patient needs and emerging behavioral health trends. Patient and Community Support: Conduct screenings and assessments, and guide patients and families in accessing community resources and referrals. Provide crisis intervention when needed. Documentation and Confidentiality: Maintain accurate, detailed records of patient care in the EMR, ensuring confidentiality and adherence to professional standards.

Qualifications: Masters degree in Counseling or related field. Active LPC license in the state of Michigan. Experience in behavioral health therapy and patient care. Strong communication and collaboration skills.
