Requirements: ABOS Board Certified or TRULY Board Eligible in Orthopedic Sports Medicine (must become certified within 5 years) REQUIRED no exceptions Valid IL & IA license or Active IMLC Willing to Obtain Licenses - REQUIRED Strong arthroscopic and open sports surgery skills REQUIRED, shoulder, knee, hip PREFERRED Hold current and unrestricted state and federal authority to prescribe and administer medications as necessary or appropriate to the field of practice - REQUIRED DEA - REQUIRED BLS Certification and maintain recertification - REQUIRED Clean malpractice and license history HIGHLY PREFERRED / PREFERRED Does the provider require visa support No J-1 or H-1B (Note in Additional Submission Details) REQUIRED Submit Self-Query NPDB pulled within 30 days from submission at time of presentation - REQUIRED BLSBoard Certified Board Eligible
