Job Details: Schedule: Monday–Friday, 8:00 AM–4:30 PM Location: Lexington, KY Float / Coverage Area: Fayette, Jessamine, Franklin, Woodford, Anderson & Mercer Counties Job Type: Travel Contract Contract Length: 13 Weeks Pay Range: $50.00 to $54.00/hour* Mileage Reimbursement: between Patient visits Productivity: 30 points/week On Call / Weekend: None Dress Code: Business casual or scrubs (gray, black, or blue) *Pay offered is based on experience, expertise, credentialing, and education.
Duties: Provide occupational therapy assessments and treatments to patients in their homes. Perform OASIS assessments and follow-up/treatment visits. Develop and implement individualized plans of care to improve patients' functional independence. Document patient progress accurately and coordinate care with the interdisciplinary healthcare team. Provide safe, patient-centered care throughout the assigned service area.
Requirements: Education: Occupational Therapy degree from an accredited program License(s): Active Kentucky Occupational Therapist (OT) License Certification(s): Current CPR certification Years of experience: Home Health experience required Skills/Specialties/Technology: HCHB, OASIS, Home Health OT, strong knowledge of home health processes and documentation Interested?
