A growing home care and hospice organization that treats patients throughout the Bronx.
📍 Location:
Bronx, NY – 100% field-based🕘 Schedule: Monday–Friday, 9:00 AM–5:00 PM📅 Weekend Coverage: Some weekend coverage required💰 Compensation: Based on experience🎓 Tuition repayment + amazing benefits This role provides psychosocial support to hospice patients and their families across home care, nursing home, and inpatient settings. The Social Worker will be an integral member of the interdisciplinary hospice team, developing individualized plans of care, supporting patients and families through the hospice and bereavement process, and coordinating with clinical and facility staff.
Requirements:
NYS LMSW or LCSW – requiredMSW from an accredited school of social work Healthcare social work experience preferred Open to new graduates with hospice/palliative care internship experience Strong communication, organization, and listening skills Comfortable working independently in a field-based role Ability to work effectively as part of an interdisciplinary team
