Hospice Home Health Nurse Manager (RN)

Gentiva is seeking a Hospice Patient Care RN Manager to lead our home-based hospice nursing team. In this role, you will oversee patient care delivery, ensuring compassionate, clinically excellent end-of-life care in patients’ homes. You’ll supervise and mentor RNs and clinical staff, complete and review assessments, and guide individualized care plans. Partner with an interdisciplinary team to manage pain and symptoms, support families, and uphold regulatory and quality standards. Conduct comprehensive patient assessments and oversee individualized care plans. Coordinate interdisciplinary care with physicians, social workers, and chaplains. Ensure compliance with hospice regulations, policies, and clinical standards. Monitor quality metrics and drive continuous improvement in patient outcomes. Provide clinical guidance, mentoring, and training to nursing staff. Support patients and families with education, counseling, and end-of-life planning. Manage scheduling, caseloads, and resource allocation for efficient operations.
