What You'll Do:
- Provide clinical leadership and oversight for home health nursing teams
- Ensure OASIS accuracy, timeliness, and Medicare compliance
- Coach and develop clinicians to improve documentation, quality, and outcomes
- Lead case conferences and care coordination efforts
- Monitor and drive performance metrics, quality scores, and regulatory readiness
- Collaborate with branch leadership to ensure efficient, patient-centered operations? What You BringActive RN license (New Mexico or compact)
- Home health experience (required)
- Strong working knowledge of OASIS documentation and regulations
- Leadership experience (Clinical Manager, Supervisor, Lead RN, or similar)
- Knowledge of Medicare guidelines and compliance standards
- A passion for mentorship, accountability, and quality improvement?
Preferred Qualifications:
Experience with Homecare Homebase (HCHB) Proven success improving STAR ratings or quality measuresBackground managing high-volume home health caseloads or teams
