PHYSICAL THERAPY ASSISTANT

Professional Case Management is seeking a dedicated Physical Therapy Assistant to join our Healthcare & Medical Services team. In this in-home role, you will support licensed Physical Therapists by carrying out treatment plans, assisting with therapeutic exercises, documenting patient progress, and educating clients and families. You’ll work with individuals who have complex medical needs, including former energy workers, helping them improve mobility, function, and quality of life. Implement individualized physical therapy treatment plans under the supervision of a licensed Physical Therapist Provide in-home therapeutic exercises, gait training, transfer training, and functional mobility activities Educate clients and family members on home exercise programs, safety strategies, and use of assistive devices Observe, measure, and document client progress, functional status, and response to treatment in the medical record Communicate changes in client condition and treatment response to the supervising Physical Therapist and interdisciplinary team Assess home environments for safety concerns and recommend basic modifications or adaptive equipment within scope Adhere to all professional, regulatory, and company policies, including infection control and patient privacy standards Participate in care planning discussions, case conferences, and ongoing coordination of services Manage daily schedule and travel to client homes in an efficient, professional manner Engage in continuing education and company-provided training to maintain and enhance clinical competencies Physical therapy treatment implementation Gait and transfer training Therapeutic exercise instruction Functional mobility training Home safety assessment Patient and family education Use and adjustment of assistive devices Vital signs monitoring and basic clinical assessment within PTA scope Electronic medical record documentation Time management and scheduling Interdisciplinary care coordination Knowledge of home health regulations and safety protocols Infection control in home settings Pain management techniques within PTA scope Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current state license or certification as a Physical Therapist Assistant (PTA) Graduation from an accredited Physical Therapist Assistant program Current CPR/BLS certification Valid driver’s license and reliable transportation for in-home visits Ability to pass background check and health/drug screenings
