Job Title
Primary duties include:
  • Conducting a comprehensive assessment of families' strengths and needs
  • Participating in family service planning meetings, developing and implementing service plans with measurable goals and objectives
  • Working with families to overcome barriers and achieve improvement in functioning
  • Referring, arranging and advocating for services that a family needs
  • Conducting home visits to families for the purpose of counseling and monitoring of services and outcomes
  • Utilizing social work tools such as but not limited to genograms, ecomaps, and financial worksheets
  • Occasional evening and/or weekend work may be required

Employment Type: Temporary
Work Schedule: Mon–Fri 8:30am - 5:15pm; frequent evening and weekend events required
