Licensed Clinical Social Worker – Behavioral Health

Amergis is seeking a Licensed Clinical Social Worker to deliver compassionate behavioral health services across school, home, and facility-based settings. In this role, you will complete psychosocial assessments, develop and implement treatment plans, provide individual and group therapy, coordinate care with interdisciplinary teams, and connect patients and families with community resources. You will maintain detailed clinical documentation, uphold strict compliance and ethical standards, and support diverse populations while contributing to a mission-driven, patient-focused care environment with opportunities for growth. Conduct comprehensive psychosocial and behavioral health assessments for patients and families. Develop, implement, and regularly update individualized treatment and service plans. Provide individual, family, and group therapy using evidence-based interventions. Deliver crisis intervention and safety planning as needed across care settings. Coordinate care with nurses, physicians, educators, and other interdisciplinary team members. Connect patients and families with community and social service resources. Maintain accurate, timely clinical documentation and EHR records in line with regulatory standards. Ensure compliance with state, federal, and organizational policies, including confidentiality and ethics. Collaborate in case conferences and contribute to care planning and discharge planning. Participate in ongoing training and professional development within the Amergis network.
