What You'll Do: Conduct pre-admission and admission nursing assessments. Review medical records to determine appropriateness for admission. Coordinate admissions with hospitals, physicians, case managers, residents, and families. Facilitate a smooth transition for new residents entering our community. Complete and maintain accurate clinical documentation in compliance with regulatory standards. Communicate resident care needs to nursing and interdisciplinary teams. Provide education, support, and guidance to residents and families throughout the admission process. Collaborate with referral sources and healthcare partners to support census growth and resident-centered care.

What We're Looking For: Current Connecticut RN License in good standing. Previous experience in skilled nursing, long-term care, rehabilitation, case management, admissions, or geriatric nursing preferred. Strong clinical assessment and critical thinking skills. Excellent communication, customer service, and relationship-building abilities. Experience with electronic medical record systems.
