LCSW Healthcare Social Worker – Case Management

Kaiser Permanente is seeking an LCSW Case Manager to deliver compassionate, patient-centered care within our Community & Social Services department. In this role, you’ll assess psychosocial needs, develop treatment and discharge plans, coordinate services, and advocate for patients and families across healthcare and community settings. You’ll collaborate closely with physicians, nurses, and external agencies to ensure continuity of care, reduce barriers to treatment, and support chronic disease and behavioral health management. Conduct comprehensive biopsychosocial assessments for assigned patients Develop, implement, and revise individualized care and discharge plans Coordinate services across medical, behavioral health, and community providers Provide short-term counseling, crisis intervention, and resource linkage Advocate for patients and families to reduce barriers to care Collaborate with interdisciplinary teams to support treatment goals Educate patients and caregivers on diagnosis, treatment, and community supports Document assessments, interventions, and outcomes in EMR accurately and timely Monitor progress and adjust plans to improve health and psychosocial outcomes Participate in quality improvement and population health initiatives Clinical psychosocial assessment Care coordination and discharge planning Motivational interviewing Crisis intervention Knowledge of community resources Electronic medical record (EMR) documentation Behavioral health and SUD knowledge Interdisciplinary team collaboration Utilization management/care transitions Culturally competent practice Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current LCSW license in state of practice BLS/CPR certification (preferred) Certification in Case Management (ACM or CCM) preferred Experience with EMR systems (e.g., Epic) Background check and clearance
