Community Health Worker

Job Description: Job Description · Deliver agency health education programs for target populations

• Develop relationships with community support services for collaborative outreach and referral
• Mentor and assist in training of community-based interns
• Update and translate presentations and/or handouts, or create new ones as needed
• Schedule services, events such as health fairs, presentations, and other activities in the community
• Conduct lung-health screenings, including spirometry, blood pressure and oxygen saturation screening
• Conduct clean indoor air quality home assessments (and fall prevention for seniors)
• Provide tobacco cessation information and assistance
• Distribute respiratory equipment and lung disease remediation supplies
• Prepare progress reports and program evaluation
