Lead Advanced Practice Clinician, In-Home Care

Alignment Health is seeking an Advanced Practice Clinician for the Care Anywhere (CAW) program to deliver in-home medical and social support to seniors. You will assess patients, review medications, perform health screenings and coordinate with a broad care team to prevent hospitalizations and progression of disease. Qualified applicants hold an NP or PA license with board certification, EMR experience, and a background in geriatric care. A Master’s degree is required, and driving is expected.
