Multidisciplinary Case Manager (RN/RD/Clinician)

Premera is recruiting Case Managers (RN, RD, or Licensed MH Clinicians) to empower members through integrated case management. You will engage members, perform assessments, and develop care plans that improve health outcomes across physical, behavioral, and social determinants of health. The role emphasizes relationship-based engagement, effective communication, and evidence-based strategies to reduce avoidable acute care and gaps in care. Training is provided and growth is encouraged.
