Home Health Licensed Practical Nurse

CarePartners is seeking a Licensed Practical Nurse - Home Health to provide high-quality, patient-centered care in the McDowell area. In this role, you will deliver skilled nursing services in patients’ homes, support care coordination, and collaborate closely with an interdisciplinary team to improve community health outcomes. Responsibilities include patient assessments, medication administration, care plan implementation, and patient/family education. The ideal candidate is compassionate, detail-oriented, and committed to collaborative, coordinated care that enhances access and quality across our provider network. Provide skilled nursing care to home health patients in their residences, following individualized care plans and physician orders Perform comprehensive patient assessments, including monitoring vital signs, evaluating symptoms, and documenting clinical status in the electronic health record Administer medications, treatments, and wound care safely and accurately, adhering to all clinical protocols and regulatory requirements Collaborate closely with physicians, therapists, social workers, and other members of the care team to coordinate services and promote high-quality, efficient care delivery Educate patients and family members on disease management, medications, home safety, and self-care techniques to support independence and prevent avoidable hospitalizations Communicate changes in patient condition promptly to the care team and participate in updating and adjusting care plans as needed Support transitions of care by helping coordinate services, follow-up appointments, and community resources to maintain continuity of care Uphold CarePartners’ commitment to compassionate, patient-centered care and contribute to community health improvement through professional, respectful interactions with patients and families Maintain accurate, timely, and compliant documentation in accordance with home health regulations, payer requirements, and CarePartners policies Participate in quality improvement initiatives and follow evidence-based best practices to improve outcomes and patient satisfaction Home health nursing Wound care Medication administration Care planning Electronic health records (EHR) documentation Patient and family education IV therapy Vital signs monitoring Care coordination Time management Infection control Patient assessment Palliative and chronic disease management HIPAA compliance Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current, active LPN license in the state of practice Current Basic Life Support (BLS) certification Valid driver’s license and reliable transportation for home visits Home health or community nursing experience preferred Knowledge of home health regulations and documentation standards preferred
