Medicare Health Advocate - Member Outreach

CVS Health Corporation is seeking a Health Advocate to support Medicare members through the Healthy Outcomes Team. You will engage members via digital and phone channels to promote appropriate screenings, medications, and care coordination. In this role you’ll support HEDIS initiatives, document member interactions in the CRM, and collaborate with plans, providers, and pharmacies to close gaps in care while delivering empathetic, high-quality service.
