Physical Therapist, Home Health

Responsibilities: Perform patient evaluations and develop individualized plans of care Provide skilled physical therapy visits to patients in their homes Supervise and collaborate with Physical Therapist Assistants Educate patients and families on home exercise programs, safety, and fall prevention Complete OASIS assessments and visit documentation accurately and on time Coordinate care with nurses, physicians, and the interdisciplinary team

Qualifications: Active Physical Therapist license in Georgia Doctor of Physical Therapy (DPT) or master's in Physical Therapy from an accredited program Home health experience preferred; strong PTs from other settings are welcome to apply Current CPR/BLS certification Valid driver's license, reliable transportation, and auto insurance Ability to work independently and as part of a team.
