Aperion Care Oak Lawn seeks an MDS Assistant/Restorative Nurse (RN or LPN) to lead the restorative nursing program, coordinating between therapy and nursing to promote restorative care across the facility. Responsibilities include developing and monitoring the restorative plan, conducting assessments, supervising staff, attending care planning and QAPI meetings, ensuring documentation, and acting as Medicare Services Coordinator when needed.
