Field Nurse Practitioner

Molina Healthcare is seeking a mission-driven Field Nurse Practitioner to help members—especially those in underserved communities—access quality, cost-effective care. In this field-based role, you will conduct in-person health assessments, review medical histories and medications, identify care needs, provide health education, and coordinate referrals and follow-up with members’ care teams. You’ll document findings accurately and collaborate with providers to support better health outcomes. Success requires an active Nurse Practitioner license, strong clinical judgment, clear communication, and the ability to travel to member locations. Identify care needs and coordinate referrals and follow-up with members’ care teams. Educate members on preventive care, treatment plans, and available health services. Document assessments and care recommendations accurately and in a timely manner. Collaborate with providers and interdisciplinary teams to support quality, cost-effective care.
