Role Description:
This is a full-time on-site role for a Case Manager at Lakeside Assisted Living in Staten Island, NY. The Case Manager will be responsible for coordinating care plans, conducting assessments, collaborating with healthcare providers, advocating for patients, and ensuring quality care delivery.
Qualifications:
Case Management, Care Coordination, and Assessments skills Excellent communication and interpersonal skills Ability to advocate for patients and work in a team environment Knowledge of healthcare regulations and patient rights Bachelor's degree in Social Work, Psychology, or related field preferred Experience in a healthcare or long-term care setting is a plus Salary $60k to $80K negotiable based on qualifications and experience.
