Signify Health, part of CVS Health®, is seeking a Full‑Time Physician to provide In‑Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care. You’ll be primarily traveling – up to 100% of the time – across a defined coverage area, potentially with overnight hotel stays, car rentals or flights as required. Holding multiple state licenses will be required, as dictated by business needs.
Responsibilities:
Bring your heart into every visit, joining a national network of purpose‑driven clinicians dedicated to improving health outcomes.
Help receive millions of people nationwide with the goal of connecting them to the right care for their unique needs.
Work with Signify Health as a clinician colleague, fulfilling the responsibilities of the specified role.
Conduct virtual visits as needed.
Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products (e.g., spirometry, DEE, bone density screening).Travel locally and within licensed states.
Qualifications:
Active, unrestricted license(s) in coverage area(s).Able to obtain and maintain multi‑state licenses in coverage areas, as required. Ability to perform duties safely in diverse home environments.

Pay Range:
