What the work involves: Case Management Social WorkerThis is a regular PRN position, with caregivers working days between 8:00 a.m. – 4:30 p.m. Coordinate the care and services of select patients. Intervene with patients who have complex psychosocial and discharge planning needs. Conduct detailed psychosocial assessments and coordinate with community resources and supportive counseling. Offer crisis interventions to patients and families with psychosocial needs and facilitate the transition of care for high-risk patients facing barriers to care.
