Job Title: PT Home Health Therapy/Rehabilitation Location: Chesapeake, VA Setting: Hospital Pay Range: 2751.40 - 2888.97 Schedule: 08:00 - 17:00 (M-F; weekend for Ortho cases only; PT every 4th or 5th weekend) Contract Length: 13 weeks Candidate Type: Traveler Requirements: 1 year experience required; VA PT State License Required - CAN BE PENDING!!!!; CPR Required; Chesapeake will not consider candidates with any license or background hits. Responsibilities: Plan and administer medically prescribed physical therapy treatments. Provide education and information to patients and caregivers. Participate in required discharge planning. Complete all required documentation according to policy. Direct patient participation in selected tasks to restore, reinforce, and enhance performance.
