Home-Based Palliative Care NP: Compassionate Leader

Empatia, part of Gentiva, seeks a compassionate Palliative Care Nurse Practitioner to deliver advanced home-based palliative care in collaboration with the Palliative Medical Director. You will perform comprehensive assessments, develop individualized care plans, prescribe as permitted by state law, and coordinate with families and the care team. This role requires autonomy, strong communication, and willingness to participate in on-call rotations.
