2nd Family in Colorado is seeking a part-time Registered Nurse Care Coordinator to support clients in Longmont, Loveland, Greeley and Johnstown. The position offers remote work from home with required in-home assessments, and you must be based in Colorado to serve clients locally. As an RN Care Coordinator, you will conduct initial consultations, 90-day reassessments, supervise CNAs/GNAs, and coordinate the care plan to ensure smooth delivery of services.
