Medical Director of Hospice and Palliative Medicine

SSM Health is seeking a full-time Regional Clinical Medical Director to join their growing Hospice and Palliative Medicine team in Wisconsin. In this role, you will make a meaningful impact by shaping the future of palliative care and supporting a compassionate clinical team. You will deliver comprehensive, patient-centered care across multiple locations in the Wisconsin Region. Practice Info Supports inpatient, outpatient, and home-based community visits across Fond du Lac, Ripon, Waupun, Baraboo, Columbus, Madison, Janesville, and Monroe Partners collaboratively with 2 Physicians, 11 Advanced Practice Providers, and 15 dedicated support staff, including Registered Nurses, Social Workers, Chaplains, and Medical/Scheduling Office Assistants Shift & Schedule Full-time Responsibilities.2 FTE Palliative Medical Director duties, .2 FTE Hospice Medical Director duties, and .6 FTE dedicated to Palliative Care Outpatient and Telemedicine Care Collaborating provider for Hospice & Palliative Care APPsLead program initiatives Set quality and safety metrics Interdisciplinary team - Collaboratively work with Administrative Leaders, Clinical Staff, and Service LinesHelp develop and grow the Regional Hospice & Palliative Medicine Program
