Position Summary:
The Chronic Disease Coordinator has a key role on the Chronic Disease Team with the responsibility of coordinating patient support services and patient education for both on-site and remote monitoring programs that support chronic disease management at Lawndale Christian Health Center. This position will report to the Population Health Department.
Qualifications:
- Knowledge of medical terminology
- Demonstrates excellent communication skills and is able to communicate clearly to patients and staff
- Enthusiasm and ability to establish goals and develop action plans with patients
- Ability to foster relationships with and between patients
- Demonstrates effective interpersonal relationships and customer service skills
- Comfortable in either small group or large group settings
- Strong ability to independently multi-task and work well in a fast-paced, team environment
- Demonstrates problem-solving skills and able to follow protocols and established processes
- Able to be detail-oriented, think critically, troubleshoot, analyze processes, and develop corrective actions for process improvement
- Strong organizational and time management skills
- Computer literacy and experience is required including proficiency in Microsoft Office Suite
- Experience working with electronic medical records
- Able to work a flexible schedule, including evenings
- Must possess a valid driver's license

- Required Knowledge, Experience, or Licensure:
- Bachelor's degree required.
- Client Services Experience recommended.
- Bilingual in English and Spanish is a plus.
