Shift: Day 5x8-Hour (08:00 - 16:30)
Start Date:
ASAPFloating Expectation: Be ready to float to various facilities! Experience Requirement:1 full year of acute care experience is REQUIRED.Previous Charge experience is preferred. Patient Ratios: Manage a caseload of 20-23 patients. Weekend Requirement: Variable 5-day workweek with operations 7 days per week. Days off may vary and are not guaranteed to be back-to-back.
Licensure: CA state license is required. Compact license must be held at submission.
Qualifications
Certifications: BLSLCSW* / MSW
Skills:
- Patient care coordination
- Case management
- Discharge planning
- Knowledge of community resources
- Patient Details:
- Age Groups: Adults and Geriatrics
- Daily Census: 25 patients
- Position Insights:
- Work with a dedicated team of RNs and MSW/LCSWs performing discharge planning.
- Collaborate with medical staff, insurance case managers, and Providence UM case managers to ensure smooth transitions to the appropriate care settings.
- Engage with patients of varying ages and conditions.
- Important Notes:
- Must commit to working alongside fellow clinicians to deliver high-quality care.
- Flexibility to float between sister facilities may be required.
