Active WV RN License required
Productivity: 30 points per week
Coverage area: Monongalia, Preston, Tucker, Grant, and Wetzel counties
Required experience:
- Home Health, HCHB, and OASISFirsthand experience with patient assessments, care planning, case management, and coordination of care
- Responsible for medication management, physician communication, documentation, staff supervision, and patient education
- Contract position, onsite
