As a Licensed Practical Nurse LPN at Cloverdale Rehabilitation & Wellness Center, you will play a vital role in providing high-quality patient care to our patients and their families.
Responsibilities:
- Provide direct patient care, including vital signs, wound care, and medication administration
- Conduct initial patient assessments, develop and implement individualized care plans, and evaluate patient progress
- Collaborate with interdisciplinary healthcare teams to ensure comprehensive patient care
- Communicate effectively with patients, families, and healthcare professionals
- Maintain accurate and detailed patient records
- Participate in quality improvement initiatives to enhance patient outcomes and care
Requirements:
- Current LPN license in the state of Alabama
- Strong clinical skills, with attention to detail and commitment to providing exceptional patient care
- Ability to work effectively in a fast‐ paced environment, prioritizing multiple tasks and responsibilities
- Strong communication and interpersonal skills, with the ability to work collaboratively with diverse healthcare teams
