Home Health Education & Outreach Specialist

Excela Care Home Health & Hospice is seeking a Home Health Education Specialist to expand our reach by building relationships with referral sources and educating the community about our care services. You will travel to healthcare partners and represent Interim Healthcare professionally to promote timely hospice care. The role requires providing education, advocating for patients and families, and collaborating with clinical teams to ensure smooth transitions from referral to admission.
