Wound, Ostomy, Continence Registered Nurse

Hartford Healthcare is seeking a Wound, Ostomy, Continence Registered Nurse to provide expert assessment, treatment, and education for patients with complex wound, ostomy, and continence needs. In this role, you will develop and implement evidence-based care plans, perform advanced wound and ostomy procedures, and collaborate with an interdisciplinary team to optimize healing and quality of life. You will educate patients, families, and staff on prevention, product use, and self-care. Perform comprehensive wound, ostomy, and continence assessments Develop and implement individualized, evidence-based care plans Provide advanced wound and ostomy treatments and product selection Educate patients and families on self-care and prevention strategies Collaborate with physicians and interdisciplinary teams to coordinate care Document assessments, plans, and outcomes in the EMR accurately and timely Monitor healing progress and adjust treatment plans as needed Provide staff education and serve as a clinical resource for WOC care Promote infection prevention and patient safety best practices Participate in quality improvement initiatives related to wound and ostomy care Wound assessment and staging Ostomy care and management Continence assessment and management Advanced wound dressings and therapies Negative pressure wound therapy Infection prevention and control Patient and family education Interdisciplinary care coordination Electronic Medical Record (EMR) documentation Evidence-based clinical practice Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active RN license WOCN or CWOCN certification preferred or obtained within specified timeframe BLS certification
