Home Health Occupational Therapist - $10k Sign-On & Vehicle

CenterWell Home Health is seeking an Occupational Therapist to assess and retrain patients in their homes, helping improve daily living and functional independence. You will collaborate with physicians and the care team to plan, implement and document therapy, and coach patients and families toward safe home reconditioning. This full-time role requires an active OT license, a valid driver's license, and reliable transportation to serve our service area.
