Hospice Registered Nurse Case Manager

Join Addus Family company as a Hospice RN Case Manager and provide compassionate, in-home end-of-life care. You’ll manage a caseload of hospice patients, conduct comprehensive assessments, and develop individualized care plans that support comfort, dignity, and family needs. Collaborate closely with social workers, aides, chaplains, and physicians to coordinate services, manage pain and symptoms, and ensure high-quality, compliant care. Manage a caseload of hospice patients in home and community settings Conduct comprehensive nursing assessments and ongoing patient evaluations Develop and update individualized hospice care plans Administer medications and perform skilled nursing interventions, including pain and symptom management Educate patients and families on disease process, medications, and end-of-life care Coordinate care with physicians, social workers, chaplains, aides, and other interdisciplinary team members Document care accurately and timely in EMR in compliance with hospice regulations Participate in case conferences and on-call rotation as assigned Advocate for patient comfort, dignity, and goals of care Ensure adherence to safety, infection control, and regulatory standards Hospice nursing Pain and symptom management Care planning Wound care IV and medication administration Patient and family education Electronic medical record (EMR) documentation Interdisciplinary care coordination Home safety assessment Regulatory and compliance knowledge (hospice) Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active RN license in state of practice BSN preferred BLS certification Hospice & Palliative certifications preferred (e.g., CHPN) Valid driver's license and reliable transportation Experience with EMR/EHR documentation
