Hospice Licensed Practical Nurse

Provide skilled nursing care to hospice patients in the Cincinnati area, following the plan of care and applicable nursing practice standards. Assess patient needs, manage pain and symptoms, educate patients and caregivers, document services, and coordinate care with the interdisciplinary team. Provide direct patient care in accordance with the State Nurse Practice Act. Implement nursing care based on patient assessments and the plan of care. Document patient services accurately and promptly. Assess patients and educate families and caregivers about diagnoses and care. Coordinate hospice services and report patient needs to the interdisciplinary group. Provide pain and symptom management and evaluate responses to treatment and medication.
