Private Duty Home Health LPN/LVN

Aveanna Healthcare is seeking compassionate Private Duty LPN/LVNs to provide one-on-one skilled nursing care in patients’ homes. You will deliver bedside care, including assessments, medication administration, trach/vent and wound care, and monitoring of chronic and acute conditions for pediatric and adult patients. Collaborating with families and the clinical team, you’ll follow individualized care plans, document accurately, and support safe, comfortable care in the home. Provide one-on-one skilled nursing care to pediatric and adult patients in the home Perform ongoing assessments and monitor chronic and acute conditions Administer medications and treatments per physician orders Provide tracheostomy, ventilator, and respiratory support as assigned Deliver wound care, nutrition, and personal care within scope of practice Document care accurately in accordance with agency and regulatory standards Collaborate with clinical supervisors, therapists, and family members Educate patients and caregivers on disease management and home care Follow infection control and safety protocols in the home setting Participate in ongoing training and competency evaluations Patient assessment Wound care Medication administration Tracheostomy care Ventilator management Care planning documentation Infection control IV and enteral therapy Electronic health records (EHR) Caregiver and family education Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active LPN or LVN license in state of practice Current BLS/CPR certification Pediatric or adult home health experience preferred Knowledge of tracheostomy and ventilator care preferred
