Inpatient Rehabilitation Case Manager

Cardinal Hill Rehabilitation Hospital, part of Encompass Health, seeks a Case Manager to coordinate care for inpatients recovering from complex conditions. You will assess patient needs, develop individualized discharge and care plans, collaborate with physicians, therapists, and nurses, and secure services and equipment to support safe transitions home or to the next level of care. This mission-driven, patient-centered role requires strong communication, insurance and resource navigation skills, and a passion for helping patients regain function and independence. Assess patients’ rehabilitation and psychosocial needs from admission through discharge Develop and update individualized care and discharge plans Coordinate services with physicians, therapists, nurses, and community providers Manage insurance authorizations and facilitate utilization review Educate patients and families on goals, resources, and post-discharge options Arrange home health, equipment, and community resources for safe transitions Advocate for patients to support optimal functional outcomes Document assessments, plans, and progress in the EMR accurately and timely Case management Discharge planning Utilization review Interdisciplinary care coordination Electronic medical records (EMR) Insurance authorization Patient and family education Care planning Rehabilitation knowledge Communication and documentation Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current RN, LCSW, or related clinical license preferred ACM, CCM, or similar case management certification preferred CPR/BLS certification Knowledge of rehabilitation and post-acute care regulations
