Part-Time Home Health Occupational Therapist

Canyon Home Care & Hospice LLC in Gilbert, AZ is seeking a Part Time Occupational Therapist to provide intermittent home-based OT under physician orders and supervision. You will evaluate patients, develop and revise plans of care, and coordinate with physicians and families. Experience and licensure requirements include state OT licensure, ACOTE-accredited education, and NBCOT eligibility, with two years of experience preferred. CPR certification is required.
